Stel offers a Vitals Hub that is simple for patients, simple for care teams and simple for budgets. Stel’s Vitals Hub seamlessly transmits patient vitals data from the home directly into the care team’s dashboard or electronic health records. Our device agnostic Vitals Hub passively pairs with bluetooth enabled devices like scales, pulse-ox’s, blood pressure cuffs, and glucometers, and, transmits the data via cellular networks in real time right to care teams.
Stel’s Vitals Hub does not require wifi or internet, device pairing, cell phones or tablets to deliver a scalable remote patient monitoring solution for our clients. Building an RPM program with Stel is as easy as patients plugging in the VItals Hub, and taking their vitals as directed. Our low touch, seamless approach to RPM allows providers to integrate RPM into their existing workflows without the need for additional platforms or workflows for care teams to navigate.
Stel supports RPM programs in hospital systems, chronic care management programs, FQHCs, as well as primary care and specialty community practices. As the line continues to blur between the exam room and the living room, providers must double down on their ability to provide high quality virtual care beyond the pandemic. The Stel Vitals Hub gives providers the ability to monitor their patients’ vitals data remotely or during telehealth visits – meaningfully informing care decisions and achieving desired outcomes. Stel’s Vitals Hub simplifies remote monitoring for both patients and providers by providing a high tech, low touch approach to collecting and transmitting vitals data.
